A Directive to Physicians allows your 18+ year old child to plan ahead by instructing their doctor on when to administer, withhold, or withdraw life-sustaining treatment if they are diagnosed with a terminal or irreversible condition and can no longer make decisions on their own.

You will need a lawyer to make major changes or add to the documents (we cannot do that for you). We can connect you to a lawyer who has specific experience with your legal documents to help you. Our process allows us to get your documents to your lawyer immediately in a format where they can make changes quickly and efficiently.
